Whether the individual lives independently in their own home, in an adult family home, or in a community-based residential facility, they have the option to come in one day a week, five days a week, or on an as needed basis. Every circumstance is different. Contact us to find the one right for you.

We offer an array of day services:

  • Safety and Supervision
  • Assistance with Personal Care (meals, bathing, etc.)
  • Activities of Daily Living (ADL) Skills Building
  • Recreational Activities (both in-house and in the community)
  • Range of Motion (ROM) Exercises

Prevocational Opportunities

Job opportunities are provided for those who are looking to earn money or build work skills in a supervised setting. The jobs are paid at piece and sub-minimum hourly rates per licensing from the Department of Labor. Our main contract is to complete all recycling and shredding for the entire Clark County Rehabilitation and Living Center campus.

Transportation

Transportation to and from the programs can be provided by family, home staff, or a transportation agency. This is a service we can also provide at an additional cost.

Funding

Services can be funded privately by the individual or by another source such as managed care organizations, aging grants, etc.

ABOUT OUR DAY SERVICES

Our day services are provided through our adult day care and community outreach programs. Depending on the services needed and the method of payment, individuals may be in one or both of these programs.

Adult Day Care - Willow Ridge is licensed by the state of Wisconsin. There are three Willow Ridge locations in Clark County. Two of them are located on the Clark County Rehabilitation and Living Center campus, and the third is in Neillsville.

Community Outreach is a division of Clark County Rehabilitation and Living Center and is located within the same building. Its primary focus is to provide prevocational services accompanied by day habilitation programming as requested.
